UPDATE: I think I actually figured out the problem and a solution. I went to my Discord app, went to App Settings > Notifications > 'Integrate calls with Phone app' and switched it off. It seems that this was causing the issues. Discord calls may not be integrating all that well with the iPhone unfortunately.
